Gabapentin for Cats: Dosage, Side Effects, and the Kidney Disease Risk

November 06, 2026

A cat that cannot stand, will not respond, is breathing slowly, or has repeated jerking movements of the head or body after gabapentin needs a vet or emergency clinic now. Gabapentin is cleared by the kidneys, so a cat with kidney disease can build up much higher levels from a normal dose. If your cat swallowed someone's human gabapentin — especially the liquid, which usually contains xylitol — call your vet, the ASPCA Animal Poison Control Center (888-426-4435) or Pet Poison Helpline (855-764-7661).

The short answer: Gabapentin is a human nerve-calming drug that vets prescribe off-label for cats, most often as a single dose 90 minutes to 2 hours before a vet visit and as a daily medicine for arthritis and nerve pain. A typical pre-visit dose is about 20 mg/kg — one 100 mg capsule for most cats; pain doses are lower, around 5–10 mg/kg every 8–12 hours. It peaks in about an hour, has a half-life of about 3.6 hours, and the sleepiness and wobbliness had worn off within 8 hours in the main vet-visit trial. The part most pages skip: gabapentin leaves the body through the kidneys, and in an Ohio State study 92% of cats with kidney disease given 10 mg/kg had higher dose-adjusted levels than healthy cats given 20 mg/kg. It also lowered blood pressure by a median of 12 mmHg. Older cats — the ones most likely to get it for arthritis — are the ones most likely to need a smaller dose.

What Gabapentin Is and What It Does for Cats

Gabapentin (brand name Neurontin in people) was developed as a human anti-seizure drug and is now widely used for nerve pain. It binds to a part of the calcium channels on overactive nerve cells — the alpha-2-delta subunit — and turns down the release of the chemical messengers that carry pain and excitement signals. The practical result in a cat is a calmer, sleepier, less reactive animal, and for some cats less pain.

Three facts shape how it is used in cats:

  • It is not approved for cats. Gabapentin has no FDA approval for any animal. Vets prescribe the human drug, or a compounded version, under the extra-label rules that let them use human medicines in pets.
  • Cats absorb it very well. In a North Carolina State study, about 95% of an oral dose reached the bloodstream, peak levels came at about 1 hour, and the half-life was 3.6 hours. A transdermal gel rubbed on the ear did not work well enough to recommend.
  • The kidneys clear it. In people it is eliminated almost entirely unchanged by the kidneys, and the cat data point the same way. That matters for a species in which chronic kidney disease is one of the most common conditions of old age.

For a cat, gabapentin is mostly a stress drug and a pain drug — and the cats who need it for pain are often the cats whose kidneys clear it most slowly.

What Vets Use Gabapentin for in Cats

Five uses, with very different amounts of evidence behind them.

Use How it is given Best evidence in cats What it found
Vet-visit and travel stress One dose 90 min–2 h before van Haaften 2017, 20 cats, placebo crossover Owner stress scores and vet handling scores both better than placebo; sedation common
Arthritis pain Daily, usually twice a day Guedes 2018, 20 cats aged 10+, placebo crossover About 3-fold higher odds of improvement in owner-chosen problem activities
Fearful shelter cats 10 mg/kg twice daily Eagan 2023, 32 cats from hoarding cases, double-blind Median time through a behaviour program cut in half; lower stress scores
Appetite after surgery Before and 6 h after surgery Fantinati 2020, 60 cats Ate more than placebo, similar to mirtazapine; 30% met resting energy needs vs 0%
Seizures, nerve pain Add-on, every 8–12 h Case reports and clinical experience No controlled trials in cats; used alongside phenobarbital or levetiracetam

Sources: van Haaften 2017 · Guedes 2018 · Eagan 2023 · Fantinati 2020. A 2025 systematic review of 20 feline studies reached the same overall picture: good support for pre-visit stress, moderate support for arthritis, thin support elsewhere (Laguardia 2025).

The arthritis trial has a twist worth knowing. Owners saw their cats doing the things they had struggled with — jumping up, using stairs, grooming — but the collar activity monitors showed cats moved less overall on gabapentin (39,038 vs 48,333 counts a day). The likely reason is sedation. If your senior cat seems "more comfortable" but sleeps far more, tell your vet: the dose may be higher than it needs to be. For the signs to watch in an older cat, see signs of arthritis in cats.

Gabapentin Dosage for Cats

Your veterinarian sets the dose. These are the published ranges vets work from, so you can understand the prescription — not a chart to dose from.

Purpose Published range What that means for a 10-lb (4.5 kg) cat Source
Before a vet visit or trip About 20 mg/kg once, 1–2 h before; 50–200 mg per cat About 90–100 mg — usually one 100 mg capsule van Haaften 2017; GoodRx
Chronic pain, starting dose 5 mg/kg every 12 h About 25 mg twice a day Di Cesare 2023
Chronic pain, usual range 3–10 mg/kg every 8–12 h About 15–45 mg two or three times a day Merck Veterinary Manual
Arthritis trial dose 10 mg/kg every 12 h About 45 mg twice a day Guedes 2018
Cats with kidney disease Reduced dose, set by your vet Often a fraction of the healthy-cat dose Quimby 2022

Sources: Merck Veterinary Manual · Di Cesare 2023 · GoodRx.

Two things drive the doses down in real life. The first is the capsule: gabapentin comes as 100 mg capsules and larger human tablets, so a vet who wants 25 or 50 mg will have the pharmacy compound a smaller capsule or a flavoured liquid. The second is age and kidney function, covered in its own section below. A wide published range (one review lists up to 40 mg/kg) does not mean a bigger dose is better: in cats, more gabapentin mostly means more sedation and more wobbling.

How Long Gabapentin Takes to Work and How Long It Lasts

Timeline of one pre-visit gabapentin dose in cats: peak blood level at about 1 hour, half-life about 3.6 hours, sedation and wobbliness resolved by 8 hours
One dose in a healthy cat. Kidney disease stretches every part of this timeline.
  • Onset: about 1–2 hours. Peak blood level is at roughly an hour. The vet-visit trial gave the capsule 90 minutes before the carrier came out; many vets say 2 to 3 hours, and some add a dose the night before.
  • Duration: roughly 8–12 hours. All side effects had resolved within 8 hours in the trial cats. VCA notes that effects usually stop within 24 hours but can last longer with kidney or liver disease.
  • For pain, give it time. The arthritis trial measured results over two weeks. A single dose tells you little about whether daily gabapentin will help your cat's joints.
  • Keep the cat calm after dosing. A cat that is already panicking when it gets the capsule tends to get less benefit. Dose in a quiet room and leave the carrier out of sight for the next hour.

How to Give a Cat Gabapentin

  • Capsule: pop it whole into a pill pocket or a small piece of soft treat, or pill the cat directly and follow with a little water or wet food. Do not crush loose capsule powder over a whole meal — it is bitter, and a cat that leaves half the bowl gets half the dose.
  • Opened capsule: if your vet allows it, mix the powder into a teaspoon of strong-smelling wet food or a lickable treat and make sure all of it is eaten.
  • Compounded liquid: easier for small doses and fussy cats. Ask for a cat-safe flavour (fish or chicken) and confirm with the pharmacy that it is xylitol-free.
  • With or without food: either is fine. If your cat vomits after an empty-stomach dose, give it with a small meal.
  • Missed dose: give it when you remember unless the next dose is close; never double up.

Pilling is its own skill. Our guide to giving cats oil and serving sizes covers mixing into food, and cat anxiety covers carrier training — which makes any vet-visit medicine work better.

Gabapentin Side Effects in Cats

Sedation is not really a side effect here — for a vet visit it is half the point. These are the effects reported in the feline trials.

Effect How common What to do
Sleepiness, sedation Common — the most reported effect in every trial Expected. Call your vet if your cat cannot be roused or is still heavily sedated the next day
Wobbliness (ataxia), misjudged jumps Common Keep the cat on the floor, away from stairs and balconies, until steady
Drooling, vomiting Reported in the vet-visit trial Try giving with a little food next time
Weakness, muscle tremors Reported in the arthritis trial (10 of 20 cats had some adverse effect) Report to your vet — the dose may need lowering
Lower blood pressure Median −12 mmHg after one 10 mg/kg dose Tell the clinic your cat had gabapentin if blood pressure is being measured
Myoclonus (sudden jerks of the head or body) Rare — two published cases, both with kidney disease and a seizure history Same-day vet call; do not give the next dose until you have spoken to them

The blood pressure finding is easy to miss and matters in two ways. A cat given gabapentin before a check-up may read lower than its true pressure, which could hide high blood pressure — common in older cats with kidney or thyroid disease. And a cat with already-low pressure is not a good candidate for big doses.

Chart: one 10 mg/kg dose of gabapentin lowered median systolic blood pressure in cats from 137 to 121 mmHg in healthy cats and from 155 to 129 mmHg in cats with kidney disease
Same cats, gabapentin one week and placebo the other. The drop was the same size with or without kidney disease.

Gabapentin and Kidney Disease in Cats

Search for gabapentin on cat-owner forums and you will find threads from owners of cats with kidney disease warning others off it — cats that were "out of it" for a day or more after a normal pre-visit dose. The research explains why.

  • Levels run much higher. Ohio State researchers gave 25 cats with stable stage 2 or 3 kidney disease 10 mg/kg and compared them with healthy cats given 10 or 20 mg/kg. After adjusting for dose, the kidney cats had significantly higher levels at 3 and 8 hours, and 92% of them were above the top of the healthy-cat range at double the dose. The worse the creatinine and SDMA, the higher the level.
  • Blood pressure still falls. In the follow-up blood pressure study, cats with kidney disease dropped from a median 155 to 129 mmHg after a single 10 mg/kg dose.
  • Rare neurological effects show up in this group. The only two published feline cases of gabapentin-induced myoclonus were cats with stage 2 kidney disease and a seizure history, at 11.8–16.4 mg/kg before a hospital visit.

The researchers' conclusion was not "never use it". It was that a dose reduction is likely needed in cats with kidney disease, and that this probably matters even more for daily use. If your cat has kidney disease — or is over about 10 and has not had recent bloodwork — ask your vet these three things before the first dose: what are the latest creatinine and SDMA values, has the dose been reduced for them, and should we start with a trial dose at home on a quiet day before relying on it for a vet visit.

Which Cats Should Not Take Gabapentin, and Drug Interactions

  • Cats with a known reaction to gabapentin.
  • Cats with kidney disease — not a ban, but a reduced dose and a vet conversation first.
  • Cats with a seizure disorder already on gabapentin — never stop suddenly; withdrawal can trigger seizures.
  • Pregnant or nursing cats — safety has not been established.
  • Cats that will be under anaesthesia or sedation — tell the clinic; it adds to the effect of other sedatives.

Interactions to raise with your vet: other sedatives such as trazodone, alprazolam or acepromazine (deeper sedation, sometimes intended); opioids such as buprenorphine, morphine or hydrocodone (more sedation, and morphine can raise gabapentin levels); antacids containing aluminium or magnesium (space them at least 2 hours apart, since they reduce absorption); and anything else that makes a cat drowsy, including antihistamines like Benadryl for cats and hemp or CBD products.

Gabapentin vs Trazodone, Alprazolam and Bonqat for Cats

Drug Approved for cats? What the head-to-head studies found
Gabapentin No — human drug, off-label Reference drug for vet-visit stress; also used for pain
Trazodone No — human drug, off-label In 21 cats, trazodone 5 mg/kg sedated significantly and gabapentin 10 mg/kg alone did not; the combination sedated most (Tucker 2024)
Alprazolam No — human drug, controlled In 60 cats, gabapentin 100 mg and alprazolam 0.125 mg gave comparable calming and sedation (Papageorgiou 2024)
Melatonin No — supplement In 75 cats, both gabapentin 100 mg and melatonin 3 mg improved handling; gabapentin sedated more cats (Ruviaro Tuleski 2022)
Bonqat (pregabalin) Yes — FDA-approved Nov 2023 A close chemical cousin of gabapentin. In the field study, 77% of cats improved vs 46% on placebo; side effects were mild sedation, wobbliness and lethargy

Sources: Tucker 2024 · Papageorgiou 2024 · Ruviaro Tuleski 2022 · FDA, Bonqat approval · Lamminen 2023.

Bonqat is the one to ask about if your cat only needs help for vet visits. It is a measured liquid (50 mg/mL, dosed at 5 mg/kg about 90 minutes before the trip, repeatable the next day), it went through an FDA review for exactly this job, and like gabapentin it is prescription-only and a Schedule V controlled substance. Gabapentin's advantages are cost and flexibility: it is cheap, vets have years of experience with it, and the same drug can be used daily for pain. For how gabapentin and trazodone compare in dogs, see gabapentin vs trazodone.

Gabapentin for Cats: Cost and Prescription Rules

  • Cost: generic 100 mg capsules are among the cheapest drugs a vet prescribes — GoodRx lists about $12 for 30 capsules with a discount card. Compounded small capsules or flavoured liquids cost more, and prices vary by pharmacy.
  • Prescription: always required. Gabapentin is not federally controlled in the US, but several states — including Alabama, Kentucky, Michigan, Tennessee and Virginia — classify it as Schedule V, and many more track every fill in their prescription monitoring programs. That can mean limits on refills or a new prescription each time.
  • Human gabapentin at home: do not use it. Tablets are usually 300–800 mg, far too large to split accurately for a cat, and the human oral solution typically contains xylitol, which vets avoid in pets.

Where Hemp Fits — and Where It Doesn't

We make hemp products for cats, so here is the straight answer.

Vet-visit stress
Not a substitute

There is no controlled trial showing hemp extract calms cats for a vet visit the way gabapentin did. If your cat needs pre-visit medication, use what your vet prescribes.

Alongside gabapentin
A vet conversation

No study has tested CBD with gabapentin in cats. Both can cause drowsiness, and cats process hemp differently than dogs. Tell your vet, and start one thing at a time.

Everyday wellness
Where our oil fits

For daily comfort and calm in healthy cats, a very small serving of full-spectrum hemp in food is how owners use our products. Cats need a fraction of a dog serving.

Our own clinical research was in dogs with joint discomfort, not cats, and we do not claim it carries over. For what is and is not known in cats, read is CBD safe for cats, hemp for senior cats and CBD for cat anxiety.

Questions Worth Asking Your Vet

  • What is the dose in milligrams, and how was it worked out for my cat's weight?
  • When were my cat's kidneys last checked, and has the dose been adjusted for the results?
  • Should we do a trial dose at home on a quiet day before the vet visit?
  • Is Bonqat an option for my cat's vet visits instead?
  • For daily use, how will we judge whether it is helping — and what level of sleepiness is too much?
  • If we ever stop it, how should we taper?

Frequently Asked Questions

What does gabapentin do to a cat?
Gabapentin calms the nervous system by reducing the release of excitatory signals from overactive nerves. In cats it is used off-label for three things: a single dose before a vet visit or car trip to reduce fear, ongoing pain such as arthritis or nerve pain, and as an add-on for seizures. Most cats become sleepy and a little wobbly for several hours after a dose.

How quickly does gabapentin make cats sleepy?
Gabapentin reaches its peak blood level about one hour after an oral dose in cats, and most vets advise giving it 90 minutes to 2 hours before a stressful event. The research dose in the main vet-visit trial was given 90 minutes before the cat went into the carrier. Sleepiness and wobbliness had worn off within 8 hours in that trial.

How much gabapentin can a 10 lb cat take?
Only your veterinarian can set the dose. For reference, a 10-lb cat weighs about 4.5 kg. Published pre-visit doses are around 20 mg/kg, which is roughly 90 to 100 mg for a 10-lb cat, and the common single dose is one 100 mg capsule per cat. Published pain doses are lower, around 5 to 10 mg/kg (about 25 to 45 mg) every 8 to 12 hours. Cats with kidney disease usually need a reduced dose.

What should I expect after giving my cat gabapentin?
Expect a sleepy, relaxed cat that may walk unsteadily, misjudge jumps and sleep more than usual. Some cats drool or vomit. Keep the cat indoors, off high furniture and away from stairs until it is steady again, which is usually within 8 hours. Call your vet if the cat cannot stand, is unresponsive, has jerking movements or is still heavily sedated the next day.

When should you not give gabapentin to cats?
Do not give it without a prescription, and do not use human liquid gabapentin, which usually contains xylitol. Use extra caution, and only at a vet-reduced dose, in cats with chronic kidney disease, and tell your vet about any history of seizures, low blood pressure, pregnancy or nursing, and every other medication or supplement the cat takes, including other sedatives, opioids and CBD.

Is it bad for a cat to take gabapentin every day?
Daily gabapentin is commonly prescribed for arthritis and other chronic pain, and a pharmacokinetic study found that two weeks of twice-daily dosing did not change how cats process the drug. Long-term safety data in cats are still limited, so ask for periodic kidney bloodwork, watch for sedation that builds up, and never stop daily gabapentin suddenly, especially in a cat with seizures.

How long does gabapentin last in cats?
The half-life in healthy cats is about 3.6 hours, and the sedating effect of a single dose usually wears off within 8 to 12 hours. In cats with kidney disease the drug is cleared more slowly, so the effect can be stronger and last longer. That is why chronic-pain doses are typically given every 8 to 12 hours.

Is gabapentin safe for cats with kidney disease?
It can be used, but at a reduced dose and with care. In an Ohio State study, 92 percent of cats with kidney disease given 10 mg/kg had higher dose-adjusted blood levels than healthy cats given 20 mg/kg, and levels rose with creatinine and SDMA. A separate study found gabapentin lowered blood pressure by a median of 12 mmHg. Two cats with kidney disease and a seizure history developed jerking movements after pre-visit doses.

Is gabapentin FDA approved for cats?
No. Gabapentin is a human drug that veterinarians prescribe off-label for cats. The only FDA-approved drug in its class for cats is Bonqat (pregabalin oral solution), approved in November 2023 for fear and anxiety during transport and vet visits. Bonqat is a Schedule V controlled substance given at 5 mg/kg about 90 minutes before the trip.

Can I give my cat gabapentin without a vet prescription?
No. Gabapentin is prescription-only, several US states classify it as a controlled substance, and the right dose depends on your cat's weight, kidney function and other medications. Human capsules and tablets come in strengths far too large to guess at, and human oral solution usually contains xylitol. Ask your vet for a cat-sized capsule or a compounded, xylitol-free liquid.

Can I give my cat CBD with gabapentin?
Ask your vet first. No study has tested CBD and gabapentin together in cats. Both can cause drowsiness, so adding hemp could deepen sedation, and cats process many compounds differently than dogs. If your vet agrees, start one product at a time at a very small serving so you can tell what is doing what. Hemp is not a replacement for prescribed pain or anxiety medication.

The Short Version

Gabapentin is a cheap, well-absorbed human drug that vets prescribe off-label for cats — most often one dose 90 minutes to 2 hours before a vet visit, or a small daily dose for arthritis and nerve pain. Expect a sleepy, wobbly cat for up to about 8 hours. The real risk is in older cats: gabapentin leaves through the kidneys, cats with kidney disease build up far higher levels, and it lowers blood pressure. Get bloodwork, ask for a kidney-adjusted dose, never use human liquid gabapentin, don't stop daily doses suddenly, and ask whether the FDA-approved option, Bonqat, suits your cat's vet visits.
